糖尿病眼底病变患者的实时用药监护及合理性用药分析

目的:分析糖尿病眼底病患者的用药医嘱,实施药学监护,促进糖尿病眼底病患者更加标准化用药,加强合理用药,提高糖尿病眼底病患者的生活质量,保障患者用药的有效性和安全性。方法:对2019年1月1日—2019年12月31日住院的糖尿病眼底病变患者进行药学问诊、用药宣教和出院带药教育,并对用药医嘱进行实时审核,判定用药的合理性,分析不合理用药原因,提出用药建议。结果:审核病历300份,共计1148条医嘱,包括长期用药医嘱663条,临时用药医嘱485条,其中不合理医嘱76条,占全部医嘱的6.62%。不合理医嘱包括不规范给药医嘱25条(2.18%)、不适宜给药医嘱50条(4.36%)及超常医嘱1条(0.09%)。结论:糖尿病眼底病变患者用药医嘱中不合理临床医嘱以不适宜用药占比最高。药师应对眼底病患者开展药学服务,以保证药物的安全有效。

Analysis of real-time medication monitoring and rational medication in patients with diabetic fundus diseases

Objective:To analyze the medication orders of patients with diabetic ocular fundus disease,implement pharmaceutical monitoring,promote more standardization of diabetic ocular fundus patients,strengthen rational medication,improve the quality of life of diabetic ocular fundus patients throμgh real-time pharmaceutical monitoring,and ensure the effectiveness and safety of patients′medication.Methods:In this study,patients with diabetic fundus lesions hospitalized in hospital from January 1,2019 to December 31,2019 were pharmacologically consulted,used for medical education,and discharged with medicine.Real-time audits were conducted to determine the rationality of medication,analysis of unreasonable medication,and recommendations for medication.Results:A total of 300 cases were reviewed,including 663 long-term medication orders and 485 temporary medication orders,totaling 1148.There are 76 unreasonable medical orders,accounting for 6.62%of all medical orders.Unreasonable medical orders include 25 medical orders for irregular administration,50 medical orders for unsuitable administration,and 1 extraordinary medical order.These three medical orders account for 2.18%(25 articles),4.36%(50 articles),and 0.08%(1 article)of all medical orders,respectively.Conclusion:Among the medication orders of patients with diabetic fundus disease,unreasonable clinical orders are the highest.Pharmacists should carry out pharmaceutical care for patients with fundus diseases to ensure the safety and effectiveness of the drugs.
