非酒精性脂肪肝与2型糖尿病胰岛素抵抗的关系分析

目的探讨非酒精性脂肪肝（NAFLD）与2型糖尿病（T2DM）患者胰岛素抵抗的关系。方法98例确诊为2型糖尿病的患者根据是否合并非酒精性脂肪肝分为两组，比较两组患者的一般资料、实验室血清学指标。结果98例2型糖尿病患者中，合并非酒精性脂肪肝组的体重指数（BMI）、空腹血糖（FPG）、空腹血清胰岛素（FInS）、胰岛素抵抗指数（HOMA—IR）、血清总胆固醇（TC）、三酰甘油（TG）、低密度脂蛋白（LDL—C）、血尿酸水平（uA）均显著增高；高密度脂蛋白（HDL—C）显著低于未合并非酒精性脂肪肝组。上述差异均有统计学意义（P〈0．05）。结论合并非酒精性脂肪肝的糖尿病患者存在更高的胰岛素分泌水平和更明显的胰岛素抵抗。

Relationship between Non-alcoholic Fatty Liver Disease and Insulin Resistance in Patients with Type 2 Diabetes Mellitus

Objective To investigate the relationship between non-alcoholic fatty liver （ NAFLD ） and insulin resistance in type 2 diabetes （ T2DM ） patients. Methods All of 98 cases diagnosed as type 2 diabetes were divided into two groups： T2DM combined without NAFLD group and T2DM combined with NAFLD group. The general information and laboratory serum markers were measured. Results In 98 cases of type 2 diabetes, compared with T2DM combined without NAFLD, the levels of body mass index （ BMI ）, fasting plasma glucose （ FPG ）, fasting serum insulin （ FInS ）, homeostatic model assessment of insulin resistance （ HOMA-IR ）, serum total cholesterol （ TC ）, triglycerides （ TG ）, low density lipoprotein cholesterol （ LDL-C ） and uric acid （ UA ） were all significantly increased in T2DM combined with NAFLD （ P〈0.05 ） ; the level of high-density lipoprotein cholesterol （ HDL-C ） was obviously decreased in T2DM with NAFLD, compared with T2DM combined without NAFLD （P〈0.05 ）. Conclusion T2DM combined with NAFLD had higher level of insulin secretion and more significant insulin resistance than T2DM combined without NAFLD insulin resistance.
