宫腔粘连的介入性超声诊断和治疗

目的　探讨宫腔粘连的介入性超声诊断和治疗的方法。方法　 64例不孕、闭经及周期性腹痛的患者 ,行介入性超声检查并在超声监导下行经宫颈宫腔粘连松解术。结果　经介入性超声诊断宫腔粘连 63例 (98.43 % )。 64例手术中 ,62例手术顺利 ,2例穿孔。随访 42例 ,2 1例症状消失或减轻 ,其中 4例妊娠 ,2例分娩 ;13例分解粘连组织后取出宫内避孕器、残留胎骨及妊娠囊 ;3例复发。结论　介入性超声是诊断和治疗宫腔粘连的有效方法。

Interventional Ultrasound Diagnosis and Treatment for Intrauterine Adhesion

Objective To discuss the method of interventional ultrasound diagnosis and treatment for intrauterine adhesion. Methods Sixty four patients evaluated for infertility, amenorrhea and periodic abdominal pain underwent interventional ultrasonographic examination and all patients were performed transcervical resection of adhesion (TCRA) under ultrasound guidance. Results Sixty three patients (98.43%) were diagnosed interuterine adhesion by interventional ultrasound.Sixty two out of the 64 patients were successfully performed operative hysteroscopy. In the remaining 2 patients, uteri were perforated. The study followed 42 women treated by TCRA. The symptoms were relieved or improved in 21 patients, 4 of them were later on pregnant and 2 women with normal delivery. The intrauterine device,residual fetal bone and gestational sac in 13 patients were removed after TCRA. The uterine cavity relapsed into adhesion in 3 patients. Conclusion Our study showed that interventional ultrasonography is effective method in diagnosing and treating the intrauterine adhesion.
