不同临床、病理表现的IgA肾病患者血甲状腺抗体水平分析

目的：研究不同临床、病理表现的IgAN患者血中TGA及TPOA水平。方法：以140例IgAN患者作为研究对象。根据其临床表现及病理情况分组后,观察甲状腺抗体阳性组与阴性组各项指标差异。结果：1）140例IgAN患者中,TGA或（和）TPOA阳性者共51例,占36.4%,阳性组患者血TGA水平与其肾脏系膜区IgA及补体C3免疫沉积呈正相关,P〈0.05;2）不同分组患者,尿NAG酶/肌酐、尿Alb/尿cr、血补体C3/C4、血IgG以及肾小球节段损伤、炎性细胞浸润、纤维性新月体、IgA免疫复合物沉积等指标水平,可因甲状腺抗体是否阳性存在差异,P〈0.05。结论：1）IgAN患者中甲状腺自身抗体阳性率较高且其肾脏系膜区IgA与补体C3沉积受TGA水平影响;2）单纯性镜下血尿、单纯蛋白尿患者,观察甲状腺抗体情况可能对提示肾脏病理有一定指导意义。
