蚌埠市医疗机构339份临床输血病例合理性评价

目的了解蚌埠市临床合理用血状况,促进临床合理用血。方法随机抽取蚌埠市范围内40家临床医院2011年5月-2012年12月共339临床输血病例,按内科、外科、妇产科及其它科室进行分类,并分别分析临床输注红细胞和血浆的合理性。结果红细胞输注的合理病例为31.5%,相对合理病例为31.5%,不合理为37.0%;不合理红细胞输注病例中内科为4.7%,外科为63.2%,妇产科为18.2%,其它科室为28.6%;血浆输注合理病例仅占24.2%;＂搭配血＂为67.4%,外科搭配输血占输注血浆病例的39.3%。结论蚌埠市临床医疗机构内科与妇产科对红细胞输血指征的把握较好,而外科对红细胞输血的指征把握则较差。血浆输注存在随意性,合理性较低;＂搭配血＂具有一定的普遍性,医务人员（尤其外科）临床输血的理念亟待提高。

Rationality evaluation on 339 clinical blood transfusion cases from Bengbu medical institutions

Objective To investigate and then promote the rational blood transfusion situation of Bengbu city .Methods 339 clinical blood transfusion cases from 40 medical institutions in Bengbu city dating from May 2011 to December 2012 wereselected randomly and were classified by internal medicine ,surgery,gynecology and so on.Blood transfusion rationality of thesecases was analyzed respectively.Results Rational transfusion cases for red cells accounted for 31.5%,relatively rational casesfor 31.5%,and irrational cases for 37.0%.Among the irrational transfusion cases for red cells ,internal medicine accountedfor 4.7%,surgery for 63.2%,and gynecology for 18.2%,and the others for 28.6%.Rational transfusion cases for plasmaonly accounted for 24.2%.“Match transfusion” cases for 67.4%,and “match transfusion” cases of surgery for 39.3% in allplasma transfusion cases.Conclusion Internal medicine and gynecology departments in medical institutions of Bengbu didwell in grasping erythrocyte transfusion indications ,whereas surgery departments did badly .Plasma transfusion was sort of random,and its rationality rate was relatively low.“Match transfusion” was common to some extent.It is very urgent to promoteclinical blood transfusion notion of medical workers （especially surgeons）.
