摘    要:目的探讨CT引导下空洞内注药辅助治疗耐多药肺结核的治疗效果。方法将160例耐多药肺结核患者随机分为治疗组(80例)和对照组(80例),所有患者根据药敏均采用全身抗结核治疗,治疗组CT引导下经皮肺穿刺空洞内注入药物介入治疗,每1周进行1次。然后进行比较。结果 2组痰菌阴转率在治疗4个月后的任何时期均有显著性差异(P〈0.01);治疗8个月时2组病灶有效率、空洞闭合率比较均有显著性差异(P〈0.05)。结论经皮肺穿空洞内注药,空洞内抗结核药物浓度高,作用时间长,药物对全身几乎不产生毒副作用,疗效确切,对呼吸无明显影响,创伤小,并发症少,具有较高的临床应用价值。

Percutaneous lung cavity injection of medicine for treatment of multi-drug resistant cavernous tuberculosis

Abstract:Objective To explore the CT-guided percutaneous lung cavity injection of medicine and the auxiliary therapeu -tic effect for the treatment of multi-drug resistant tuberculosis .Methods 160 cases of multi-drug resistant tuberculosis pa-tients were randomly divided into a treatment group and a control group ,80 cases for each .All the patients adopted systemic anti-tuberculosis treatment based on their susceptibility .The treatment group underwent CT guided percutaneous lung puncture with cavity injection of drug for intervention , once a week .Comparison was then followed .Results The sputum negative con-version rates of the two groups were significantly different ( P〈0.01) at any time after four months'treatment.By the 8 months,the efficiency rate for lesions and the cavity closure rate of the two groups also showed significant difference ( P〈0.05).Conclusion By percutaneous pulmonary cavity injection of medicine ,anti-TB drugs are in high concentration in lung cavity,and effect time is thus prolonged .It has the benefits of being effective in treatment ,rarely harming breathing ,small trau-ma,and fewer complications .It is worth clinical recommendation .
