扬州地区结核与非结核分枝杆菌耐药现状分析

摘    要:目的通过总结近年来扬州地区结核与非结核分枝杆菌耐药现状,为该地区肺结核病化疗选择药物提供参考依据。方法通过鉴别培养基对硝基苯甲酸(PNB)和噻吩-2-羟酸肼(TCH)做分枝杆菌菌型初筛鉴别,绝对浓度法检测耐药。结果结核分枝杆菌群分离率为94.63%(141/149),非结核分枝杆菌分离率为5.37%(8/149);结核分枝杆菌群耐多药率14.18%,总耐药率49.65%。非结核分枝杆菌耐多药率87.5%,总耐药率100%。结核分枝杆菌群以耐TH1321、SM、INH和RFP多见,非结核分枝杆菌高耐药与结核分枝杆菌群相似。结论本地区肺结核以结核分枝杆菌群为主,耐多药率及总耐药率高于全国水平,非结核分枝杆菌分离率较低,但临床应十分警惕。

Situation analysis of mycobacterium tuberculosis and nontuberculosis mycobacteria drug resistance in Yangzhou

Abstract:Objective To summarize situation of mycobacterium tuberculosis and nontuberculosis mycobacteria drug resistance, to provide the reference basis for the drug choice of tuberculosis chemotherapy. Methods Differential medium(PNB) and thiophene p-nitrobenzoic acid-2-hydroxy acid hydrazine(TCH) were done early screening to identify bacteria mycobacterium type,absolute concentration method of detection of drug resistance. Results Tuberculosis separation rate of mycobacterium was 94.63%(141/149),the separation rate of nontuberculosis mycobacteria was 5.37%(8/149),group of multidrug-resistant mycobacterium tuberculosis rate was 14.18%,49.65% of the total resistance. Of multidrug-resistant nontuberculosis mycobacteria rate was 87.5%, the total resistance rate was 100%. Resistance of mycobacterium tuberculosis in TH1321, SM,INH and RFP,nontuberculosis mycobacteria was similar to mycobacterium tuberculosis group of high drug resistance. Conclusion Tuberculosis in Yangzhou mainly is mycobacterium tuberculosis,the region of multi-drug resistant rate and total resistance is higher than the national level,the separation rate of nontuberculosis mycobacteria is low,but should be very alert.
