胆道支架置入联合适形放疗治疗恶性梗阻性黄疸的临床观察

摘    要:目的探讨胆道支架植入术联合三维适形放疗对恶性梗阻性黄疸的临床疗效,评价其应用价值。方法15例失去手术机会的恶性梗阻性黄疸患者放置胆道支架后接受肿瘤局部三维适形放疗。结果治疗后所有患者黄疸、临床症状均明显缓解,同时取得8例PR、5例SD的近期肿瘤控制率,并发症主要为肝区疼痛,经对症处理症状缓解,未见严重并发症发生。结论初步观察表明胆道支架置入治疗恶性梗阻性黄疸具有微创、有效的优点,而适形放疗可减少支架置入后再狭窄阻塞的可能的特点,是一种安全可行的方法。

Abstract:Objective To investigate the clinical efficacy of biliary stenting plus three-dimensional conformal radiotherapy for malignant obstructive jaundice and evaluate its application value.Methods Fifteen patients with malignant obstructive jaundice who had lost surgical opportunities were performed tumor partial three-dimensional conformal radiotherapy after placement of biliary stents.Results After the treatment,all the patients' jaundice and clinical symptoms were significantly alleviated with a recent tumor control rate of PR in eight patients and SD in 5 ones.The main complication was liver pain and no other serious complications occurred.Conclusion Biliary stenting,as a effective treatment for malignant obstructive jaundice,has the virtue of minimal invasion,and conformal radiotherapy can reduce stent restenosis.
