iNOS、CD34、PCNA在肛周尖锐湿疣皮损中的表达及其意义

摘    要:目的:探讨诱导型一氧化氮合酶(induciblenitricoxidesynthase,iNOS)在尖锐湿疣(CA)中的表达及皮损中细胞增殖和血管增生的相关性。方法:采用免疫组化方法检测了17例尖锐湿疣组织及10例正常包皮组织中iNOS、CD34、增殖性细胞核抗原(proliferatingcellnuclearantigen,PCNA)的表达。结果:(1)CA皮损中iNOS表达较其在正常人表皮中的表达显著增强(P<0.05),正常人表皮中仅基底层弱阳性表达,而CA皮损区iNOS表达为基底层至角质层下方。(2)CA皮损真皮乳头微血管数较正常人表皮显著增多(P<0.05)。(3)CA皮损中iNOS的表达水平与PCNA标记指数(PCNAlabelindex,PLI)、血管密度计数(microvesseldensity,MVD)均呈显著正相关(R=-0.5910,P<0.01;R=-0.782,P<0.05)。结论:iNOS在尖锐湿疣皮损中表达异常,高水平的iNOS表达可能促进尖锐湿疣细胞增殖及真皮乳头血管形成,参与其发病。

Abstract:Objective: To study the expression of inducible nitric oxide synthase (iNOS), cell proliferation and angiogenesis and the relationship between them in condyloma acuminatum (CA). Methods: The expressions of iNOS, CD34 and proliferating cell nuclear antigen (PCNA) in skin lesions of 17 CA cases and in 10 normal prepuce skin were examined with immunohistochemistry staining (SP). Results: The expression of iNOS was significantly higher in lesional epidermis of CA than in epidermis of the controls ( P <0.05). The weak expression of iNOS was found in basal cell layer in a large number of healthy controls. However, strong expression was observed in entire epidermis of CA except the horny layer. The microvessel density (MVD) was significantly higher in dermal papilla in the lesion of CA than that in dermal papilla of controls ( P <0.05). There was a significantly positive correlation between the levels of expression of iNOS in lesion epidermis and PCNA label index (PLI) of patients (R=-0.5910, P <0.01), and the significantly positive correlation was also found between the expression levels of iNOS and MVD of patients (R=-0.782, P <0.05). Conclusion: The expression of iNOS is markedly increased in lesional epidermis of patient with CA. The higher expression of iNOS may increase the cell proliferation and vascularization, and thus take part in the pathogenesis of CA.
