立体定向导航下微创穿刺治疗丘脑出血

摘    要:目的评估立体定向导航下微创穿刺在治疗丘脑出血中的临床应用价值,以及探讨丘脑出血临床治疗方案。方法对照组20例,为2011年6—12月期间收治的丘脑出血患者,单纯行微创术。观察组24例,为2012年1—6月期间收治的丘脑出血患者,均在立体定向下行微创术。结果术前年龄、性别、患侧经统计分析均无差异。观察组有效率明显高于对照组。观察组并发症:颅内感染、肺部感染、偏瘫、再出血发生率明显低于对照组(P〈0.05)。结论针对脑深部出血,应用立体定向导航技术,术中在导航棒的指引下,精准地标记病灶范围,从而对脑内深部重要区域最大程度减少创伤,达到真正意义上的微侵袭手术。此方法值得临床大力推广。

MINIMALLY INVASIVE OPERATION IN THE TRATMENT OF PATIENTS WITH THALAMUS HEMOR- RHAGE UNDER STEREOTACTIC NAVIGATION

Abstract:Objective To evaluate the clinical applicational value of minimally invasive puncture with stereotactic navigation for the treatment of thalamic hemorrhage patients, and to investigate the clinical treatment of thalamic hemorrhage. Methods Totally 20 patients with thalamic hemorrhage in the control group were treated with mini- mally invasive surgery from June 2011 to December 2011, while 24 eases of thalamic hemorrhage in the experi- mental group were treated with minimally invasive operation under stereotactic navigation from January 2012 to June 2012. Results There were no statistical differences in age, gender and the affected side between two groups. The efficiency of experimental group was significantly higher than that of the control group, and the complications of experimental group including intracranial infection, pulmonary infection, hemiplegia and re- bleeding rate were significantly lower than that of the control group (P~0.05}. Conclusion For deep brain hemorrhage, application of the stereotactic navigation with the guidance of navigational bar in operation can mark precisely the range of le- sions, thus to reduce trauma to deep important intracerebral area, to achieve minimally invasive operation on real significance. This method is worthy of clinical application.
