Abstract:An 8-month-old girl presented with classical vitamin D deficiency rickets, secondary hyper-parathyroidism and hyperchloraemic acidosis. Renal acidification and bicarbonate titration studies showed the patient to have a proximal renal tubular acidosis. Rickets, secondary hy-perparathyroidism and proximal tubular acidosis were corrected by administration of calcium and vitamin D. A causal relationship between hyperparathyroidism and renal proximal tubular acidosis is suggested.
