摘    要:目的:探讨同期肺减容在单侧肺移植术治疗终末期慢性阻塞型肺气肿病人的疗效.方法:本呼研所胸外科1例46岁男性,严重肺气肿,肺功能重度障碍病人,给予单侧供体肺裁剪后移植,并同期对侧肺减容手术.结果:术后患者恢复良好,肺功能明显得到改善:术后20个月,肺功能改善明显,第1秒用力肺活量(FEV1)由15%提升至42%,6 min步行距离由不能耐受提高到可慢步行约620 m,基本生活自理.结论:对于终末期慢性阻塞型肺气肿,单侧肺移植结合同期肺减容术具有良好的疗效.

Abstract:Objective:To discuss the efficacy of simultaneous lung volume reduction in single-lung transplantation for end-stage chronic obstructive pulmonary disease patients.Methods: A 46-year-old male patient with severe emphysema and seriously impaired lung function admitted to our Thoracic Surgery Department was performed simultaneous ipsilateral lung volume reduction and contralateral lung grafting.Results: The patient experienced good post-surgical course and improved lung function.Lung function test were evaluated 20 months after the transplantation demonstrated significant improvement,with FEV1 increasing to 42% from 15%,and 6-minute walk distance increasing to 620 meters from previously intolerance.The patient was self-administered.Conclusion: simultaneous lung volume reduction in the single-lung transplantation for end-stage chronic obstructive pulmonary disease patients appeared to be significantly effective procedure.
