腹腔镜下子宫动脉缝扎在子宫次全切除术中应用体会

摘    要:目的:探讨腹腔镜下子宫次全切除术中子宫动脉缝扎方式的手术效果及减少并发症的临床意义。方法:为286例患者行子宫动脉缝扎后再行子宫次全切术,观察手术时间、手术平均失血量、术后疼痛发生率、术后使用抗生素的时间,术后体温、肛门排气时间,术后下床活动时间及并发症发生等情况。结果:286例患者于腹腔镜下缝扎子宫动脉后,行子宫次全切除术,手术平均用时(59.6±33.2)m in,术中平均失血量(60.1±23.2)m l,术后疼痛发生率47%,术后使用抗生素时间(3.4±2.1)d,术后体温(37.8±1.33)℃,肛门排气时间(21±8.77)h,术后下床活动时间(23.2±1.6)h,术后住院(4.2±1.8)d,术后性生活恢复时间(25.1±4.2)d,无1例中转开腹。随访2~24个月无手术并发症发生。结论:腹腔镜下子宫动脉缝扎后行子宫次全切除术安全、有效,患者康复快,能减少并发症的发生。

Abstract:Objective:To explore the clinical significance of the laparoscopic uterine artery ligation in subtotal hysterectomy.Methods:The uterine artery ligation combined with subtotal hysterectomy by laparoscope was performed on 286 patients,and the operation time,the average blood loss,the post operative pain incidence,the time of using antibiotic,the post operative body temperature,the time of passage of gas,the time of out of bed and in hospital,the time of having sex and the complication were observed.Results:The above indexes were(59.6±33.2)minutes,(60.1±23.2)ml,47%,(3.4±2.1)days,(37.8±1.33)℃,(21±8.77)hours,(23.2±1.6)hours,(4.2±1.8)days,and(25.1±4.2)days.Followed up from 2 to 24 months,no complication occurred.Conclusions:Subtotal hysterectomy by laparoscope after the uterine artery ligation is safe and effect with few complication.
