皮肤扩张耳郭再造术

［ 背景］ 耳郭再造方法多种多样．［ 病例报告］１９９８ 年４ 月收治１ 例双耳烧伤缺损患者．先做耳后皮肤扩张器植入术，扩张完毕后，掀起皮瓣和筋膜瓣，将雕塑的软骨支架植入于两瓣之间．术后皮瓣因远端血运障碍而坏死、软骨支架外露，经过适当处理后愈合．再造另一侧耳时注意到皮瓣血运问题，没有出现并发症．［ 讨论］ 应为皮肤扩张提供足够的皮瓣以利于包裹软骨支架、显露外形，手术效果令人满意．

Skin expansion and auricle reconstruction

BACKGROUD The method of reconstructing the ear is diverse. CASE REPORT In 1998, a patient with defect of bilateral ears by burning was treated by the two stage operation, i.e. first skin tissue expansion and later reconstruction of the ear. After skin expansion, the skin and fascial flaps were raised, the carved rib cartilage framework was inserted between them. After operation, the distal part of skin flap was necrosis and the cartilage framework was exposed.The wound healed through appropriate measure. The other ear was reconstructed without any complication since we were careful in handling the skin and fascial flaps. DISCUSSION Skin expansion supplies enough skin flap to wrap the cartilage framework and displays the contour of the ear. The effect of surgery is satisfactory.
