实施广泛肠切除应注意的几个问题

目的探讨如何减少广泛肠切除,避免短肠综合征的发生,保护肠道功能.方法总结分析28例实行广泛肠切除患者的临床资料.结果切除空肠6例,空、回肠7例,回肠6例,回肠、回盲部5例,结、直肠4例,平均切除肠管长度120.4 cm.28例中治愈25例,死亡3例.术后并发症:切口裂开2例,肠管再次坏死2例,慢性腹泻5例,体重轻度下降10例.结论及时影像学检查,采用合理灵活的手术方式,珍惜肠管,可以减少肠管的丢失.

Issues worth noticing in the performance of extensive intestinal resection

Objective To decrease the extention of intestinal resection,avoid the short-bowel syndrome and protect the intestinal function.Methods Clinical data of 28 cases were analyzed who had undergone extensive intestinal resection.Results The resection was conducted on sections of jejunum(6 cases),jejunum-ileum(7 cases),ileum(6 cases),ileum-ileocecum(5 cases),and colon-rectum(4 cases).The average length of the resected intestinal canal was 120.4 cm.Among the 28 cases,25 were recovered with 3 cases of death.Post-operative complications included disruption in incision(2 cases),renecrosis of intestines(2 cases),chronic diarrhea(5 cases)and slight loss of body weight(10 cases).Conclusion Timely image inspection,proper operational measures and endearment of intestines may contribute to the reduction of loss of intestinal canals.
