北京市院外急救死因分析及急救对策

目的分析院外急救中死亡者的死因，为提高院外急救质量提供依据。方法对北京急救中心2006年1月1目～12月31日出诊中死亡患者的死因进行回顾性分析，死因分类按照ICD-10标准进行。结果这段时间内北京急救巾心出诊院外死亡2939例，循环系统疾病为首要死因，其中心源性猝死居全死因第一位（占46．89％）。呼救一车辆到达现场的平均反应时间为12．8min。结论心源性猝死为院外死亡病因之首，应采取综合措施提高院外猝死的复苏存活率。

Death causes in pre-hospital emergency medical service in Beijing and analysis of the strategy

Objective To investigate the cause of out-of-hospital death, in order to improve the prehospital emergency medical service （EMS）. Methods The clinical data of the deaths during pre-hospital care by Beijing Emergency Medical Center during the period of 1 January 2006 to 31 December 2 939 were analyzed retrospectively. The causes of death were identified according to the ICD-10 codes. Results 69 846 patients received pre-hospital care during this period of which 2 939 patients, 1 550 males and 1 389 females with a male/female ratio of 1.12：1, died. 1 921 of the 2 939 patients （65.36%） died of circulation system diseases, 406 （13.81%） died of tumors, and 259 （8.81% ） died of respiratory system diseases. Circulation system disease was the first cause of death, among which sudden cardiac death was the leading cause （accounting for 46.89%）. The mean interval between call to the EMS and arrival of EMS personnel was 12.8 min. Cone I us i on Sudden cardiac death is the leading cause of out-of-hospital death. Comprehensive measures should be taken to improve the resuscitation outcome for out-of-hospital sudden death.
