移植肾长期存活者抗HLA抗体的研究

目的 研究肾移植术后患者抗HLA抗体与移植肾长期存活的关系.方法 选择2008年前已行移植肾手术的652例肾移植患者,于2009年10月-2010年4月检测群体反应性抗体（PRA）,而后每年连续观察肾功能.结果 652例肾移植患者中,抗HLA抗体阳性患者81例,占12.42％（81/652）,移植肾功能下降或丧失患者71例,占87.65％（71/81）.PRA阴性患者571例,占87.58％（571/652）,移植肾功能下降患者44例,占7.71％（44/571）.PRA阳性与阴性对肾移植患者移植肾功能的影响具有统计学差异（χ^2 =306.64,P＜0.001）.结论 抗HLA抗体可能是引起移植肾慢性失功的重要因素之一.

Study on Anti-HLA Antibody by Long-Term Survival Patient with Renal Transplant

Objective To study the relationship between HLA antibody and transplant renal long term survival in transplant patients. Methods Panel reactive antibody （PRA） was test from October,2009 to April,2010 in 652 patienls who received transphmt before 2008 ,continue renal function was observed every year. Results Among the 652 transplant patients,81 pa ticnts showed H1.A antibody positive, 12.42% （81/652）, in which, 71 patients showed renal funetion loss or decrease, 87.65%（71/81）. 571 patients showed HLA negative,87.58% （571/552）,in which,44 patients showed renal function de crease,7.71 5（44/571）. The difference were statistics between PRA positive and negative on transplant renal function（χ^2= 306.64,P〈（0. 001） o Conclusion HLA antibody would be one of the important factors that cause chronic transplant renal function loss.
