摘    要:目的观察CT引导下微波消融联合125I粒子植入术治疗恶性胸膜间皮瘤的有效性及安全性。方法对16例恶性胸膜间皮瘤患者行CT引导下微波消融联合125I粒子植入术,观察治疗效果、不良反应,记录随访所见。结果16例治疗均成功,癌痛及胸腔积液均明显缓解;术中未见穿刺部位、微波消融区及粒子植入区大出血等严重并发症及3级以上放射性损伤,未出现粒子迁移,术中及围手术期无死亡病例。术后6个月总有效率68.75%(11/16)、局部控制率87.50%(14/16)。随访12个月期间5例死亡,但无粒子植入相关死亡病例。结论微波消融联合125I放射性粒子植入治疗恶性胸膜间皮瘤安全、有效。

Abstract:Objective To observe the efficacy and safety of CT-guided microwave ablation combined with 125I seed implantation in treatment of malignant pleural mesothelioma. Methods A total of 16 patients with malignant pleural mesothelioma underwent CT guided microwave ablation combined with 125I seed implantation. The therapeutic effect and adverse reactions were observed, and follow-up results were recorded. Results The operations were successful in all 16 patients, and pain and pleural effusion relieved after treatments. Six months after operation, the total effective rate was 68.75% (11/16), and the local control rate was 87.50% (14/16). No serious complication such as massive hemorrhage in puncture site, microwave ablation area or seed implantation area, nor radiation injury above grade 3 or particle migration occurred. At the end of 12-month follow-up, 5 patients died, but there was no implantation related death. Conclusion Microwave ablation combined with 125I radioactive seed implantation was safe and effective in treatment of malignant pleural mesothelioma.
