动态监测超敏C反应蛋白和D二聚体在重症慢性阻塞性肺病急性发作期中的临床意义

目的 探讨动态监测超敏C反应蛋白（hs-CRP）和D二聚体（D-D）在重症慢性阻塞性肺病急性发作期（AECOPD）临床诊疗中的意义.方法 对50例AECOPD重症患者入院即时、治疗后3d血中的hs-CRP和D-D进行回顾比较,分析其患者病情变化和转归的关系.结果 50例AECOPD重症患者均经积极治疗后,明显好转43例（86％）,死亡7例（14％）.明显好转组血中hs-CRP和D-D在治疗后有明显的下降,与治疗前比较有统计学意义（P＜0.05）；死亡组血中hs-CRP和D-D在治疗后呈持续上升趋势,与治疗前比较无统计学意义（P＞0.05）.结论 血中hs-CRP和D-D的变化水平与AECOPD重症患者的病情演变有相关性,其变化可反映疾病的进展及严重程度.

Clinical Significance of Dynamic Monitor Use High-Sensitivity C-Reactive Protein and D-dimer in Patients with Severe Acute Exacerbations of Chronic Obstructive Pulmonary

Objective To explore the value of dynamic monitor of the high-sensitivity C-reactive protein（hs-CRP） and D-dimer（D-D） in patients with severe acute exacerbations of chronic obstructive pulmonary disease（AECOPD） Methods Blood sample from venous were collected in 50 critical patients with AECOPD on admission,3 rd after treatment and were analyzed retrospectively.The level of hs-CRP and D-D were measured and compared according to the condition changes and prognosis.Results With active treatment,43cases improved evidently（86％） while 7 cases died（14％）.The level of hs-CRP and D-D decreased significantly in the improved evidently group as compared with those before treatment,the difference was significant （P ＜ 0.05）.In the death group,the level of hs-CRP and D-D kept elevating,there was no significant difference between pre and post treatment （P ＞ 0.05）.Conclusion The level of hs-CRP and D-D are related with the progression of critical severe AECOPD.Dynamic monitor of the hs-CRP and D-D ould reflect the severity and development situation with severe AECOPD.
