以躯体化症状为主诉的儿童情绪障碍41例分析

目的　对综合性医院儿科中情绪障碍躯体化患儿的发病因素、症状表现、就诊经过、治疗结果等进行探讨 ,以引起综合性医院临床儿科医生对心理障碍躯体化症状的重视。方法　选取 1996～ 2 0 0 1年上海徐汇区中心医院儿童心理门诊中有躯体化症状的情绪障碍患儿 4 1例作为研究对象 ,调查分析其病史 ,由专业心理医生作出诊治方案 ,并评估疗效。结果　 73 2 %的患儿起病后半年才到心理门诊就诊 ,仅有 17 0 %的患儿是在非心理科的医生的建议下去看心理门诊的。情绪障碍的发病除了有外在刺激因素外 ,与患儿本身的情绪有关。躯体化症状以心血管症状、神经症状、消化道症状、泌尿生殖系统症状为主。经心理医生的治疗 97%以上的患儿得以康复。结论　综合性医院临床儿科医生对情绪障碍躯体化症状的认识辨别能力急需提高。

Children emotional disorder with somatization as chief complaint:analysis of 41 cases

Objective By exploring the pathogenic factors,symptoms,diagnosis and treatment of somatic emotional disorder,it is hoped that clinicians in general hospitals pay special attention to symptoms of somatic psychogenia.Methods A study was conducted on 41 subjects chosen from the cases in the psychiatry department of our hospital from 1996 to 2001.Results Of them,73.2% of the patients didn't go to see the psychiatrist until half a year after the onset of the disease,only 17.0% took professional consultations on the advice of a non-psychiatrist doctor.In addition to the external stimulating factors,the onset of the psychogenia was related to the patient's emotional factors.Somatic symptoms mainly involved cardiovascular,neural,digestive and urogenital system.Over 97% of the patients recovered after psychiatrists treatment.Conclusion Clinical pediatricians in general hospitals need to improve their understanding of somatization of emotional disorders.
