快速半定量绒毛膜促性腺激素测定在宫外孕诊治中的应用

目的研究绒毛膜促性腺激素(chorionicgonadotropin,HCG)快速半定量测定的可靠性及其在宫外孕诊治中的应用价值.方法31例生育年龄的妇女,因可疑宫外孕或不规则阴道出血行HCG快速测定板法半定量测定及放射免疫分析法(RIA)定量测定和阴道B超检查,部分患者行诊断性刮宫.结果11例手术治疗,并证实为宫外孕,其中10例HCG定量测定最小值为11550 U/L,最大值为22 238 U/L,半定量测定均＞10 000 U/L,18例定量测定在6 540U/L以下,半定量测定则为5 000左右或以下,有很好的一致性;3例定量测定分别为1 807 U/L、2 067 U/L及3 030 U/L,半定量测定判定为5 000 U/L～10 000 U/L;半定量测定与定量测定结果的一致性为90.3%.14例宫外孕保守治疗成功,6例HCG＜100 U/L,未确诊为妊娠,不治自愈.结论HCG快速测定板法半定量测定,快速简便,可信性高,可随时检测,非常有利于宫外孕的早期诊断和及时指导治疗.

Application of fast semiquantitative HCG test in ectopic pregnancy clinic

Objective To evaluate the validity of fast semiquantitatae HCG test in ectcpic pregnancy clinic. Methods Semiquantitative HCG test and HCG radioimmunoassay (R1A) were taken in 31 patients with irregular vaginal bleeding or suspected hectopic pregnancy. Their results were compared. Results In 90. 3 % of patients the results of semiquantitative HCG were compactable with that of HCG RIA, of which 11 patients underwent timely surgical procedure, 14 patients healed with conservative treatment of methotrexate (MTX). The rest 6 patients with quite low level of HCG recovered without any treatment. Conclusion Semiquantitative HCG test is a simple, fast and useful method in assisting early diagnosis and guiding treatment selection of ectopic pregnancy.
