乳腺导管内癌及其微浸润与乳腺浸润性导管癌的超声及病理特征

摘    要:目的 分析乳腺导管内癌(DCIS)、乳腺导管内癌伴微浸润(DCIS-MI)与乳腺浸润性导管癌(IDC)的超声及病理特征.方法 回顾性分析2014年9月至2016年9月入住我院实施手术且术后病理检查确诊为乳腺癌的170例患者的超声检查结果及病理特征.结果 IDC组患者的淋巴转移率为45.85%,明显高于DCIS组患者的3.92%和DCIS-MI组患者的4.26%,差异均有统计学意义(P<0.05);DCIS-MI组患者在病灶形态、边缘、周边毛刺、回声类型、有无钙化及周边组织血流情况方面与DCIS组、IDC组比较差异均有统计学意义(P<0.05);DCIS-MI组患者粉刺型比例为76.71%,明显高于DCIS组的46.81%和IDC组和58.33%,差异均有统计学意义(P<0.05);DCIS-MI组患者的C-erbB-2阳性率为62.75%,明显高于DCIS组的57.45%和IDC组的41.67%,差异均有统计学意义(P<0.05).结论 DCIS、DCIS-MI和IDC三者存在不同的超声表现及病理特征,临床上可以此为依据进行疾病的诊断及分型,以便为患者提供更加准确有效的治疗.

Abstract:Objective To analyze the ultrasonographic and pathologic features of breast ductal carcino-ma (DCIS), breast ductal carcinoma with microinvasion (DCIS-MI) and breast invasive ductal carcinoma (IDC). Methods The ultrasonographic and pathologic features of 170 patients, who admitted to our hospital and diag-nosed for breast cancer by postoperative pathology from September 2014 to September 2016, were retrospectively ana-lyzed. Results The lymph node metastasis rate of the IDC group was 45.85%, which was significantly higher than 3.92% of DCIS group and 4.26% of DCIS-MI group (P<0.05). Compared with patients in the DCIS group and IDC group, there were significant differences for the DCIS-MI group in terms of lesion morphology, edge, peripheral burr, echo type, calcification and peripheral blood flow (P<0.05). The acne type ratio in the DCIS-MI group was 76.71%, which was significantly higher than 46.81%of the DCIS group and 58.33%of the IDC group (P<0.05). The C-erbB-2 positive expression rate of in the DCIS-MI group was 62.75%, which was higher than 57.45%of the DCIS group and 41.67%of the IDC group (P<0.05). Conclusion DCIS, DCIS-MI and IDC have significantly different ultrasonic mani-festations and pathological features, which can be used as a basis for the diagnosis and classification of diseases, provid-ing more accurate and effective treatment for patients.
