25-羟维生素D3与2型糖尿病患者下肢动脉粥样硬化病变的相关性研究

摘    要:目的探讨2型糖尿病者25-羟维生素D_3[25(OH)D_3]水平与下肢动脉粥样硬化病变(LEAD)的相关性。方法选择2017年7~12月在我院住院的2型糖尿病患者共120例,按有无下肢动脉粥样硬化病变将患者分为两组,LEAD组(66例)和非LEAD组(54例),测定总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、糖化血红蛋白(HbA1c)、C反应蛋白(CRP)、25(OH)D_3,比较两组间指标的差异,并分析LEAD的影响因素。结果 LEAD组与非LEAD组相比,有较大的年龄、较长的糖尿病病程、较高的吸烟率、SBP、HbA1c、LDL-C和CRP,而25(OH)D_3水平明显降低(P0.05)。二分类Logistic回归分析显示,病程、HbA1c及25(OH)D_3水平是2型糖尿病患者LEAD患病危险增加的独立危险因素。结论在2型糖尿病患者中,低25(OH)D_3水平与LEAD的发生有关。

Study on the correlation between 25 -hydroxyvitamin D3 and lower extremity arterial disease in patients with type 2 diabetes
Abstract:Objective To investigate the correlation between the level of 25-hydroxyvitamin D3[25(OH)D3] and lower extremity arterial disease(LEAD). Methods 120 patients with type 2 diabetes in the inpatient department of our hospital from July to December 2017 were selected and divided into group LEAD(66 patients) and group non-LEAD(54 patients) according to the occurrence of LEAD. The levels of total cholesterol(TC), triglyceride(TG), high-density lipoprotein cholesterol(HDL-C), low-density lipoprotein cholesterol(LDL-C), glycosylated hemoglobin(HbA1c), C-reaction protein(CRP) and 25 (OH)D3 were assessed. The differences of indicators between two groups were compared and the influence factors of LEAD were analyzed. Results Compared with group non-LEAD, patients in group LEAD aged older,were with longer course of diabetes and higher smoking rate(P<0.05). The levels of SBP, HbA1c, LDL-C and CRP in group LEAD were significantly higher while the level of 25(OH)D3 was lower than group non-LEAD(P<0.05). The results of binary logistic regression showed the course of diabetes, the course of diabetes, levels of HbA1c and 25(OH)D3 were the independent risk factors of increasing risk of LEAD in patients with type 2 diabetes. Conclusion In patients with type 2 diabetes, low level of 25(OH)D3 is related with the occurrence of LEAD.
