联合运用MRI多种成像技术在胰腺肿块鉴别诊断中的价值

目的 研究联合运用MRI多种成像技术在胰腺肿块鉴别诊断中的价值。资料与方法搜集经手术病理证实的胰腺癌和胰腺炎性肿块患者共47例，采用的MRI技术分别为FLASH T1WI、TSE T2WI、T1WI＋FS、T2WI＋FS、Gd-DTPA动态增强扫描和MRCP，分析其MRI表现。结果 T1WI、T2WI、T1WI＋Fs、T2WI＋FS、Gd—DTPA动态增强扫描以及MRCP对26例胰腺癌的检出率分别为53．8％、50．0％、73．1％、76．9％、88．5％、84．6％，对21例胰腺炎性肿块的检出率分别为52．4％、57．1％、71．4％、71．4％、81．0％、85．7％。结论 T1WI＋FS、T2WI＋FS、Gd—DTPA动态增强扫描和MRCP成像技术对胰腺肿块的定位、定性准确率较高，联合运用MRI多种成像技术是鉴别诊断胰腺肿块的理想方法。

Diagnostic Value Between Pancreatic Cancer and Inflammatory Pancreatic Mass by Varied Techniques of MRI

Objective To study the diagnostic value of varied techniques of MRI for pancreatic tumor.Materials and Methods MRI findings of pancreatic cancer and inflammatory pancreatic mass in 47 cases were analyzed retrospectively. All the cases were proved by operation and pathology. FLASH T_1WI, TSE T_2WI, T_1WI FS, T_2WI FS, dynamic contrast of Gd-DTPA and MRCP sequences were used, MRI findings were analyzed.Results The accuracy of T_1WI, T_2WI, T_1WI FS, T_2WI FS, dynamic contrast of Gd-DTPA and MRCP for 26 cases of pancreatic cancer were respectively 53.9%, 50.0%, 73.1%, 76.9%, 88.5%, 84.6%. The accuracy of T_1WI, T_2WI, T_1WI FS, T_2WI FS, dynamic contrast of Gd-DTPA and MRCP for 21 cases of inflammatory pancreatic mass were respectively 52.4%, 57.1%, 71.4%, 71.4%, 81.0%, 85.7%.Conclusion T_1WI FS, T_2WI FS, dynamic contrast of Gd-DTPA and MRCP examinations have a high accuracy for the location and the qualitative of pancreatic mass, which are ideal differential modalities between pancreatic cancer and inflammatory pancreatic mass.
