磁敏感加权成像在中枢神经系统病变的应用价值

摘    要:目的探讨磁敏感加权成像(SWI)对中枢神经系统常见病变的诊断价值。方法回顾性分析90例经临床或病理证实的中枢神经系统疾病的SWI图像与常规MRI图像。结果 40例脑血管病变(包括20例脑血管病合并出血及20例脑血管畸形),SWI均清晰显示病变边界及形态异常血管,尤其对海绵状血管瘤显示敏感,而常规MRI仅清楚显示24例,另外16例陈旧性出血病变未能显示;12例脑外伤,SWI均显示损伤部位及合并出血,而常规MRI有4例共8处病变未显示;38例脑肿瘤,SWI较常规MRI序列对病变边界及瘤内出血显示更加清楚。结论 SWI序列在显示脑血管病,脑血管畸形,脑创伤,颅脑肿瘤内出血较常规MRI敏感,对中枢神经系统常见病的诊断及治疗有较高应用价值。

Abstract:Objective To assess the clinical utility of susceptibility weighted imaging(SWI) sequence for diagnosing common diseases of the central nervous system(CNS).Methods SWI findings of 90 patients with clinically or pathologically proved CNS diseases were retrospectively analyzed and compared with those of conventional MRI.Results In 40 cerebrovascular disease patients who comprised 20 cerebral tumors and 20 cerebrovascular malformations,SWI sequence clearly showed the margin and shape of the abnormal vascular lesions especially the cavernous hemangiomas but conventional MRI only showed the lesions of 24 cases while other 16 cases of old hemorrhagic lesions were not showed.In 12 craniocerebral trauma patients,SWI showed the injuried sites and complicated hemorrhage in all patients but conventional MRI only showed 8 lesions in 4 cases.In 38 cerebral tumor patients,WSI could more clearly show the lesional margin and intratumor hemorrhage than conventional MRI.Conclusion SWI sequemce in showing the cerebrovascular disease,cerebrovaseular malformation,craniocerebral trauma,cerebral tumor and intratumor hemorrhage is more sensitive than conventional MRI so that it has important utility in the diagnosis and treatment of common CNS diseases.
