两种三联疗法治疗消化性溃疡近期疗效观察

目的：为观察新三联（奥美拉唑,阿莫西林,呋喃唑酮）与标准三联（奥美拉唑,阿莫西林,甲硝唑）疗法治疗幽门螺杆菌（HP）阳性消化性溃疡的近期临床疗效对比.方法：经胃镜证实有活动性消化性溃疡且HP检测阳性的60例患者,随机分为治疗组和对照组,每组各30例.治疗组应用新三联疗法：奥美拉唑（PPl）20mg,1次/日,清晨空服,连用4周;阿莫西林500mg,4次/日;呋喃唑酮200mg,2次/日;连用2周.对照组用标准三联疗法：PPI和阿莫西林用法同新三联,甲硝唑400mg,2次/日,饭后服用,连服2周.疗程结束后一周复查胃镜.结果：60例均完成治疗周期,溃疡愈合：治疗组28例（93.3%）,对照组27例（90%）;HP根除率：治疗组27例（90%）,对照组26例（86.7%）,两组相比均无显著性差异,P＞0.05.毒副反应：两组均为轻度消化道反应,均未影响治疗.结论：新三联疗法与标准三联疗法治疗消化性溃疡及HP阳性根除率近期疗效相当,副反应可耐受,在消化性溃疡HP阳性对甲硝唑耐药情况下,可选用新三联疗法作为治疗的一线方案或补救方案.

Clinical effect of two kinds of triplex treatment which used to treat peptic ulcer

Objective:To contrast the short-term clinical effect of new triplex method (Omeprazole,Amoxicillin,Furazolidone) and standard triplex method (Omeprazole,Amoxicillin,Metronidazole) which used to treat active Peptic ulcer and Helicobacter pylori(Hp) infection.Methods:We selected 60 cases of active peptic ulcer and Hp infective pations which have been verified by fibergastroscope.Those pationts have been divided randomly into two growps.Each group has 30 cases. The treatment group was treated by new triplex.Method:Omeprazole(PPI)20mg,Once a day, eating by empty stomach in the morning, and consecutive oral 4 weeks;Amoxicilline 500mg,4 times a day;Furazolidone 200mg,twice a day,and consecutive oral 2 weeks.The contrast group was treated by standard triplex method:The usage of PPI and Amoxicilline is same as the treatment group.Metrondozole 400mg,twice a day,eating after meal.And consecutive oral 2 weeks.7 days later after a treatment course was finished,each pationt will receive a check of fibergastroscope again. Results:The healing rate of ulcer was 93.3% in treatment group and 86.7% in contrast group.The eradicating rate of Hp was 93% in treatment group and 86.7% in contrast group.There was no significant difference between the two groups (P>0.05).Toxic and adverse effects:Both of the two groups had a little gastrointestinal reaction,but it did not affect treatment.Conclusion:It is similarly about the healing rate of peptic ulcer and Hp infection in both of the two groups,and the adverse effect can be tolerated.When the pationt had drug-resistance to Metronidazole,We can selected new triplex method as the primary scheme or rescue scheme.
