后腹腔镜与开放式两种肾蒂淋巴管结扎术治疗乳糜尿的疗效比较

目的比较后腹腔镜肾蒂淋巴管结扎术和开放肾蒂淋巴管结扎术对乳糜尿的治疗效果。方法行后腹腔镜肾蒂淋巴管结扎术37例，开放肾蒂淋巴管结扎术27倒，比较两种方法的手术时间、出血量、乳糜尿消失时闻、肠功能恢复时间、下床活动时间、住院时间的差别。结果在手术时间、出血量、肠功能恢复时间、下床活动时闻、住院时间方面，后腹腔镜肾蒂淋巴管结扎术优势明显（P〈0．05）；在乳糜尿消失时间方面，两组间比较差别无统计学意义（P〉O．05）。结论后腹腔镜下肾蒂淋巴管结扎术治疗乳糜尿是一种安全有效的治疗方法，具有创伤小，并发症少，住院时间短，恢复快等优点，值得临床推广。

Comparative Study of Retroperitoneoscopic Renal Pedicle Lymphatic Disconnection and Open Surgery for the Treatment of Chyluria

Objective To evaluate the clinical effects of ligating renal pedicle lymphatic by abdominoscope through retroperitoneum and patefaction operation for treating chyluria.Methods we performed 37 patients retroperitoneoscopic ligation of renal lymphatic vessel and 20 patients open surgery. Operation time, blood loss, chyluria disappearance time, intestinal function recovery time, get out of bed time and length of stay of them were comparatively studied.Results In the operation time, blood loss, intestinal function recovery time, get out of bed time, hospitalization time, retroperitoneoscopic disconnection of renal pedical lymphatic have obvious advantages （P〈0.05）; but between the two groups in chyluria disappearance time showed no significant differences （P〉 0.05）.Conclusion Retroperitoneoscopic disconnection of renal pedical lymphatic for the treatment of chyluria is a safe,efficient method, with advantages of mini-invasion,rapid recovery, less complications and hospital stay, worth to be extended.
