Survivin蛋白在髓母细胞瘤中的表达及临床意义

目的 探讨Survivin在成人与小儿髓母细胞瘤中表达的关系及Survivin蛋白在髓母细胞瘤与良性脑膜瘤之间表达的关系。探索Survivin与预后的关系，为临床治疗方案的选择和预后的判断提供依据。方法 采用免疫组织化学方法，分别采用半定量计分法和图像分析定量方法统计64例髓母细胞瘤组织Survivin蛋白表达的阳性率及表达水平。结果 Survivin蛋白阳性染色均定位于髓母细胞瘤细胞的胞浆内。64例髓母细胞瘤组织Survivin蛋白表达的阳性率为56．25％（36／64），而良性脑膜瘤组织中Survivin蛋白表达的阳性率为0，二者间差异有统计学意义（P〈0．001）。Survivin蛋白在成人髓母细胞瘤组织中表达的阳性率45．45％，在小儿髓母细胞瘤组织中表达的阳性率58．49％，二者表达无明显差异。Survivin蛋白阳性表达率增高与髓母细胞瘤生存率下降显著相关。结论 ①Survivin可能通过抑制细胞凋亡，促进髓母细胞瘤的恶性转化。②髓母细胞瘤患者Survivin表达的程度可能与年龄无明显相关。③Survivin蛋白的表达与预后密切相关，可能作为髓母细胞瘤预后的指标。

Expression of Survivin in Medulloblastoma Tissue and its Significance

Objective To investigate the expression of Survivin in the medulloblastoma in adults and children, and the expression of Survivin in medulloblastoma and benign meningoma. Methods Histochemistry method was used to detect the expression of the Survivin protein in 64 cases of medulloblastoma.Semi-quantitative scoring method and quantitative method by figure analysis were used to calculate the positive rate and expression level of survivin protein. Results The positive stain of survivin protein all located in the cytoplast of medulloblastoma cells. The positive rate of Survivin protein expression in medulloblastoma was 56. 25% （36/64）, while the positive rate of Survivin protein expression in the tissues of benign meningoma was zero, and there were obvious differences between them（ P 〈 0.001 ）. The positive rate of Survivin protein expression in the tissues of adult medulloblastoma was 45.45% ,while the positive rate of Survivin protein expression in the tissues of children medullobJastoma was 58.49%, mad there are no significant differences in the tissues of medulloblastoma between adults and children. It was obviously negative correlation between the positive expressive rate of survivin protein and survival rate of medulloblastoma. Conclusions 1. Survivin can improve the malignant transformation of the medulloblastoma,through the inhibition of cell apoptosis. 2. Expression of survivin protein in medulloblastoma is no significant correlation with the age of patients. 3. Survivin protein is the important marker of prog- nosis, havina the unique value of Droznosis.
