术后恒温循环式腹腔热灌注化疗预防进展期胃癌局部复发转移

目的探讨进展期胃癌患者术后行恒温循环式腹腔热灌注化疗预防局部复发和转移的疗效。方法回顾性分析郑州大学附属肿瘤医院普外科2010年收治的166例行胃癌根治术（D2）的进展期胃癌患者的临床资料，其中术后行腹腔热灌注化疗加静脉辅助化疗85例（治疗组，n=85），单纯静脉辅助化疗81例（对照组，n=81）。分析患者预后情况。结果术后1年、2年及3年治疗组局部复发转移率明显低于对照组，差异均有统计学意义（P〈0．05）；远处转移率两组差异均无统计学意义（P〉0．05）。术后1年生存率治疗组与对照组差异无统计学意义（P〉0．05）；术后2年及3年生存率治疗组明显高于对照组，差异有统计学意义（P〈0．05）。结论进展期胃癌患者术后行恒温循环式腹腔热灌注化疗可以降低局部复发率，改善患者预后。

Effect of homothermal cyclic continuous hyperthermia peritoneal perfusion chemotherapy To prevent local recurrence and metastasis in patients with advanced gastric cancer

Objective To assess the effect of prevent local recurrence and metastasis in the patients with advanced gastric cancer undergoing homothermal cyclic continuous hyperthermia peritoneal perfusion chemotherapy. Methods One hundred and sixty - six patients with advanced gastric cancer were divided into the treatment group（ n = 85 ） and control group （ n = 81 ） , all of which underwent radical gastrectomy and D2 node dissection. The patients in treatment group received homothermal cyclic continuous hyperthermia peritoneal perfusion chemotherapy and intra- venous chemotherapy after surgery, whereas those in control group only received intravenous chemotherapy. The rates of local recurrence, metastasis and survival in both groups were observed. Results The 1 - year,2 - year and 3 - year local recurrence rate after surgery in treatment group were significantly lower than control group, the differences between the two groups were significant（ P 〈 0. 05 ）. No significant differences were observed in metastasis rate between two groups（ P 〉 0. 05 ）. There was no significant difference between the two groups about the 1 - year survival rate（ P 〉 0. 05 ）. The 2 - year and 3 - year survival rate in treatment group were higher than control group, the differences between the two groups were significant （P 〈 0. 05 ）. Conclusion The local recurrence rate was descend and the prognosis was improved in patients with advanced gastric cancer undergoing homothermal cyclic continuous hyperthermia peritoneal per- fusion chemotherapy.
