小儿肠套叠空气灌肠X线诊断与复位50例分析

目的:分析空气灌肠整复小儿肠套叠的影响因素,总结经验,提高整复成功率。方法:作者回顾性分析我院1999年至2004年50例肠套叠患儿行空气灌肠,并辅以手法按摩,就其整复成功率与病程、肠套叠类型及程度、套叠肠管周围结构及其病变、全身情况及方法进行分析和讨论。结果:本文50例肠套叠分3型,其中结肠型15例(30%),回结型21例(42%),小肠型14例(28%),空气灌肠整复成功率为82%。肠套叠整复成功率随病程的延长及套叠程度的加深而降低,整复成功与患儿全身情况、套叠肠管周围结构及其病变(5例)和整复方法有密切关系。结论:综合分析影响空气灌肠整复肠套叠的各种因素,掌握好适应证和注意事项,采用综合方法,才能提高小儿肠套叠的复位成功率。

The X-ray Findings and Experience of Air Enema Reduction in Pediatric Intussusception Analysis of 50 Cases

Objective To analysis different influence factors and increase the successful rate that treat pediatric intussusception with air enema reduction.Methods 50 cases of pediatric acute intussusception were retrospectively analyzed.All these cases were treated by automatic air enema intussusception reductor and manual massage.The relationship between successful rate of air enema reduction and intussusception time,intussusception type,intussusception degree,structures around intussusception intestine,systemic situation,reduction methods were analyzed and discussed.Results 50 cases of pediatric intussusception were classified by 3 types:15 cases were colocolonic intussusception(30%),21 cases were ileocolonic intussusecption(28%),14 cases were intestinum tenue intusssusecption(28%).82% were cured.The successful rate of air enema reduction would decrease wih the longthen of intussusception time and the serious degree of intusssusecption.Other relative influential factors were systemic situation,structures around intussusception testine, the lesion itself and reduction methods.Conclusion To analysis different influence factors,master indication and use multi-methods can improve the successful rate of air enema reduction in pediatric intussusception.
