俯卧位通气治疗急性呼吸窘迫综合征患者的护理体会

目的总结重症监护室早期应用俯卧位通气治疗急性呼吸窘迫综合征患者的临床护理经验。方法择取2015年12月～2019年6月我院收治的36例ARDS患者,均在各种常规体位通气基础上再辅助俯卧位通气方式。统计分析患者俯卧位通气治疗前后p H值、动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)、氧合指数(PaO_2/FiO_2)及排痰情况。结果对急性呼吸窘迫综合征患者实施俯卧位通气后,患者p H值无变化(P0.05),PaO_2显著增高,差异有统计学意义(P0.01),PaCO_2显著降低,差异有统计学意义(P0.01),PaO_2/FiO_2显著提高,差异有统计学意义(P0.01),痰液引流量增加,差异有统计学意义(P0.01)。结论俯卧位通气应用于急性呼吸窘迫综合征患者中可取得较好的护理效果,明显改善肺通气,并促进呼吸排痰,无明显并发症和不良事件发生,值得推广。

Nursing experience of prone position ventilation in the treatment of patients with acute respiratory distress syndrome

Objective To summarize the clinical nursing experience of early application of prone position ventilation in the treatment of acute respiratory distress syndrome in intensive care unit.Methods A total of 36 patients with ARDS who were admitted to our hospital from December 2015 to June 2019 were enrolled.They were treated with the prone position ventilation based on various kinds of conventional position ventilation.The patients'' pH value,arterial oxygen partial pressure(PaO2),arterial blood carbon dioxide partial pressure (PaCO2),oxygenation index (PaO2/FiO2) and drainage before and after prone position ventilation treatment were analyzed statistically.Results After prone position ventilation in patients with acute respiratory distress syndrome,there was no change in pH value (P>0.05),and PaO2was significantly increased,with significant difference(P<0.01);PaCO2was significantly decreased,with significant difference (P<0.01);PaO2/FiO2was significantly increased,and the difference was significant (P<0.01);the sputum drainage was increased,and the difference was significant (P<0.01).Conclusion Prone position ventilation can achieve better nursing effect in patients with acute respiratory distress syndrome,obviously improve lung ventilation,and promote respiratory and drainage.No obvious complications and adverse events occur.It is worth promoting.
