颅内动脉瘤的血管内栓塞治疗

目的探讨血管内栓塞治疗颅内动脉瘤的疗效。方法回顾性分析了34例颅内动脉瘤的血管内栓塞治疗,其中3例行球囊瘤内栓塞治疗,5例行载瘤动脉球囊闭塞术,26例行GDC栓塞治疗。结果26例GDC栓塞患者中,100%栓塞16例,90~95%栓塞8例,不到90%栓塞2例;术中出现脑血管痉挛3例,脑血栓形成2例,未出现动脉瘤破裂;34例病人出院时按格拉斯哥预后评分(GOS)评价结果,良好26例,中残6例,重残2例,无植物生存及死亡病例;术后随访无再出血病例。结论由于血管内治疗技术和材料的不断进步,尤其是GDC的广泛应用,多数颅内动脉瘤可以进行有效的血管内栓塞治疗。

Endovascular embolization of intracranial aneurysm

Objective:To investigate the curative effect of endovascular embolization on intracranial aneurysm. Methods: The data of 34 patients who had been treated with endovascular embolization were reviewed retrospectively,Among these patients,3 patients were treated by embolizing the aneurysmal sac with detachable balloons,5 patients were treated by occlusing the parents artery of the aneurysms with detachable balloons,and 26 patients were treated with GDC embolization. Results: of the 26 patients being embolized with GDC,16 patients were achieved 100% occlusion,8 patients 90-95% occlusion,and 2 patients lower than 90% occlusion,2 patients suffered from cerebral embolism,3 patients suffered from cerebral vasospasm No aneurysm was ruptured during the procedure;The clinical outcomes of the 34 patients were evaluated according to the Glasgow Outcome Scale when the patients dismissed from the hospital,26 patients were good recovery,6 patients were moderate disability,2 patients were severe disability,no vegetative state and death.No rebleeding occurred in the follow-up. Conclusions: Because of the improvement of endovascular technique and materials,especially of the wide-ranging application of the GDC,most of the intracranial aneurysms can be treated effectively.
