蛋白芯片检测系统在恶性肿瘤诊断中的临床研究

摘    要:目的 评价多肿瘤标志物蛋白芯片(C-12)在健康人群的普查、肿瘤的早期诊断及监测预后中的应用,研究C-12在恶性肿瘤诊断中的临床应用价值.方法 测定分析265例恶性肿瘤患者、206例良性疾病患者和326例健康体检者血清中12种常见肿瘤标志物的水平.同时对恶性肿瘤组的54例阴性标本采用全自动化学发光免疫分析仪进行比对分析.结果 C-12对恶性肿瘤组的阳性率为79.7%,显著高于良性疾病组的35.9%和健康体检组的3.1%,差异有统计学意义(P<0.01).C-12对恶性肿瘤检测的灵敏度为81.2%,特异度为79.2%,准确率为78.1%,联合检测的阳性率均显著高于单一标志物,差异有统计学意义(P<0.01).结论 C-12检测技术具有较高的灵敏度和特异性.运用蛋白芯片技术联合检测多种肿瘤标志物可以明显提高恶性肿瘤诊断的敏感性,可应用于预后及疗效观察,同时也可以作为无症状人群的早期肿瘤普查手段之一.C-12检测系统监测病情和判断预后的价值优于诊断价值,但是用于恶性肿瘤的早期诊断灵敏度不高.

Abstract:Objective To evaluate the application of protein biochip of tumor marke(C-12) in the general investigation for healthy crowd,early diagnosis and monitor of tumor,and to study the clinical application worth in the diagnose of malignant tumor.Methods 12 kinds of familiar tumors mark level in serum of 265 malignant tumor sufferers,206 positive disease sufferers and 326 healthy check crowd was analyzed.89 feminine gender specimens of malignant tumor group were analyzed and compared using full automatic chemistry luminescence immunity analysis instrument progress at the same time.Results The positive rate(79.7%) of C-12 in the malignant tumor group was significantly higher than that in the benign disease group(35.9%) and the healthy group(3.1%)(P0.01).The sensitivity of the protein biochip of tumor marke for malignant tumor examines was 81.2%,the specificity was 79.2%,and the accuracy was 78.1%.The positive rate in the unite examination group was significantly higher than than in single examination group(P0.01).Conclusion The examination technique of tumor marke of protein biochip has higher sensitivity and particularity,which can be used for the diagnosis of malignant tumor with obviously high sensibility,and can be applied effect observation after preparing and curative,as one of earlier period tumor general investigation ways for the non symptom crowd at the same time.The value of many tumors mark protein chip detection system for monitoring the condition and criterion is better than that for worth diagnosis,but its sensitivity is not high when used for the diagnose in the earlier period of malignant tumor crowd.
