22例食管间质瘤临床分析

目的 探讨食管间质瘤(esophageal stromal tumor,EST).方法 回顾性分析22例EST患者的临床表现、内镜检查、免疫组织化学的结果.结果 EST临床症状以吞咽困难为主.22例EST患者术前3例确诊,3例复发,2例发生转移度死亡；免疫组织化学检测:CD117阳性率86.36％,CD34阳性率72.73％,Vimentin阳性率63.64％,SMA阳性率45.45％,S-100阳性率9.09％；CD117与CD34表达差异无统计学意义(P＞0.05).结论 EST术前确诊困难,最终确诊主要依靠病理检查,CD117和CD34是重要标志物；治疗仍以手术治疗为主.

Clinical analysis of esophageal stromal tumor in 22 cases

Objective To investigate the value of different clinical diagnosis methods for esophageal stromal tumors（EST）. Methods The clinical data in 22 patients with surgically and pathologically proved EST were performed the retrospective analysis on the clinical manifestations, endoscopy and immunohistochemistry detecting results. Results Dysphagia was the main symptom of EST. Among 22 cases of EST,3 cases were diagnosed before operation, 3 cases relapsed and 2 eases had metastasis and died. The immunohistochemistry detection showed that the positive rate was 86.36% for CDl17,72.73% for CD34,63.64% for Vimentin,45.45% for SMA and 9.09% for S- 100. There was no statistical difference between CD117 expression group and CD34 expression group（P〉0.05）. Conclusion The preoperative diagnosis of EST is difficult and the final diagnosis is depended on pathology. CDl17 and CD34 are the important markers. Surgical operation is mainly treatment.
