3种清洁手术围手术期抗菌药物应用调查与分析

目的促进围手术期抗菌药物的合理使用。方法回顾性调查2011年1～3月某医院出院的66例3种清洁手术患者预防应用抗菌药物的情况。结果66例手术病例中,64例预防性使用了抗菌药物,预防用药率为97%。术前0.5～2h使用抗菌药物51例,占79.69%;术后使用12例,占18.76%;1例于术前2天使用。抗菌药物预防应用时间为2～6天,平均3.66天。使用的抗菌药物涉及4类9个品种,其中一代头孢菌素57例,占89.06%。患者平均住院药品费用为1619.68元,抗菌药物费用占总药费的41.68%。结论3种清洁手术围手术期预防用药存在不合理现象,应加强监管。

Analysis of Antibiotic Applications during Perioperative Period in Three Types of Aseptic Operations

Objective To improve the prophylactic use of antibiotics during the perioperative period.Methods The case records of 66 patients undergoing three types of aseptic operations in the first quarter of 2011 were retrospectively reviewed regarding of the prophylactic use of antibiotics.Results Among the 66 cases,64 patients received antibiotics,indicating that the prophylactic use rate was 97%;About 79.69% of the cases received antibiotics within 2 hours before surgery and 18.76% did not receive any antibiotics before the surgery;one case used antibiotics 2 days before the operation,and the application time of antibiotics was 2 to 6 days with an average time of 3.66 days.In total,9 kinds of antibiotics in 4 categories were used,and 57 cases used the first generation cephalosporin,accounting for 89.06%.The patients＇ total drug cost was 1619.68 yuan on the average,of which the cost of antibiotics accounted for 41.46%.Conclusion Great attention should be paid to the serious problem of irrational use of prophylactic antibiotics during the perioperative period in hospitals.
