三种清洁手术围手术期抗菌药物预防应用调查分析

目的了解我院清洁手术围手术期抗菌药物预防使用情况。方法回顾性调查我院2009年5~9月期间273例三种清洁手术患者预防应用抗菌药物情况。结果 273份患者病历中,抗菌药物预防使用率达100%,无指征用药为81.7%;术前应用t>2h者26例(占9.5%),0.5h≤t≤2h者241例(占88.28%);术后应用t≤24h者61例(占22.3%),24h48h者170例(占62.3%);预防使用抗菌药物涉及8大类,使用最多的为头孢菌素类;存在无指征用药、抗菌药物品种选择不当、联合用药不规范、用法用量不合理、应用时间过长、更换药品无依据等不合理、不规范使用情况。结论我院清洁手术患者预防用抗菌药物存在不合理现象,应引起重视。

Survey and Analysis on the Preventive Use of Antibiotics during Perioperative Period in Three Types of Aseptic Operation

Objective To get some information about the preventive use of antibiotics during perioperative period for types of aseptic operations in our hospital.Methods The case records of 273 patients undergoing types of aseptic operations between May.2009 and Sep.2009 were revived retrospectively regarding the prophylactic use of antibiotics.Results All of the 273 patients（100%） received antibiotics,81.7% used antibiotics without indication.Of whom,26（9.5%） received antibiotics at t2h and 241（88.28%）,at 0.5h≤t≤2h before operation.61（22.3%） received antibiotics at t≤24h and 42（15.4%） at 24ht≤48h and 170（62.3%） at t48h after operation.8 categories of antibiotics were used in the 273 patients,dominating the first place were cephalosporins.The irrational and nonstandard use of antibiotics manifested as improper in the used antibiotics without indication,choice of drug variety,nonstandard in drug combination,irrational in dosage and administration,prolonged use of antibiotics,lacking basis for the change of drug variety etc.Conclusion Great attention should be paid to the serious problem of irrational use of prophylactic antibiotics in the perioperative period in our hospital.
