横行带蒂包皮瓣与阴囊L形皮瓣在一期尿道成形术中的应用

目的探讨应用横行带蒂包皮瓣与阴囊L形皮瓣法治疗尿道下裂的经验，提高一期修复尿道下裂的成功率。方法2008年1月至2009年1月间，应用横行带蒂包皮瓣法行一期尿道成形术治疗尿道下裂12例，应用阴囊L形皮瓣行一期尿道成形术治疗尿道下裂3例。结果随访6—24个月。15例患者中，一期治愈13例。术后发生尿瘘1例，经尿瘘修补术治愈。尿道外口狭窄1例，经尿道扩张后治愈。结论应用横行带蒂包皮瓣与阴囊L形皮瓣法行一期尿道成形术是治疗尿道下裂的较好术式。最佳的手术方式、认真的手术操作、合理的术后包扎是提高尿道下裂治愈率的关键。

One - stage repair of hypospsdias by utilization of trnnsverse isiand flap with L - shaped island scrotal septal flap

Objective To increase the successful rate of one- stagedurethroplasty by the application transverse island flap and the treatment of L - shaped island scrotal septal flap hypos padias experience. Meflaods From Jan 2008 to Jan 2009,12 cases of congenital hypospadias were treated with transverse island flap urethroplasty and 3 cases with L-shaped island scrotal septal flap urethroplasty. Results All cases were followed up for 6 to 24 months. Of all patients 13 cases were one - stage cured, 1 case with postoperative urinary fistula was cured by urinary fistula repair, and 1 case with urethral steno sis was cured after transurethral resection of the expansion. Conclusion The transverse island flap and the L - shaped island scrotal septal flap are the better operation treatments to hypospadias. The best surgical method, serious surgical operation and reasonable post - operative dressing is the key that improve the cure rate.
