闭合性肾损伤56例治疗分析

目的　探讨闭合性肾损伤的有效治疗方法。方法　回顾分析 5 6例闭合性肾损伤患者的治疗情况。结果　保守治疗肾损伤 42例 ,2 8例随访 3月～ 1年 ,均无并发症 ;2例重度肾脏损伤因保守治疗失败行肾切除术 ;1例保守治疗后 ,患者呈持续高血压状态 ,经肾动脉造影发现肾血管损伤而行脾—肾动脉吻合术。 11例行手术探查 ,Ⅰ类肾损伤 3例 ,Ⅲ类肾损伤 3例修补治愈 ,5例伤势较重而行肾切除术。 5 6例中 ,除 2例死于合并症外 ,其他疗效均满意。结论　动态观察保守治疗闭合性肾损伤患者的病情变化很有必要

Analysis on treatment of 56 patients with of closed injuries of kidney

Objective To exprore effective method of treatment of closed injuries. Methods The therapeutic results of 56 patients with closed injuries of kidney were retrospectively analyzed. Results In 42 patients treated conservatively,therapeutic effects were satisfactory and without any complications in the following-up period of 3～12 months.Nephrectomy was carried out in 2 patients who fails to respond to conservative measure.Persistent hypertension was found by renal artery radiography in 1 patient untill spleno-renal artery anastomosis was taken.Surgical exploration was conducted in 11 patients, 8 of them were serious renal laceration(3 repair and 5 nephrectomy).Treatment results were successful in all except two patients who died of serious injuries.Conclusions It is necessary to keep observing for conservative treatment in closed injuries of kidney.
