脑外伤合并股骨骨折后leptin对骨折愈合的实验研究

摘    要:目的研究大鼠股骨骨折合并脑外伤时血清leptin水平及骨痂组织中leptin的表达,探讨leptin与骨折合并脑外伤时骨折愈合加快的关系方法将64只SD大鼠随机分为四组:对照组、脑外伤组、骨折组和骨折合并脑外伤组各16只。建立大鼠中度脑损伤和骨折模型,分别于1、2、4、8﹑12周取血清,放射免疫法测血清中leptin水平,骨折标本摄X线片,骨痂组织作免疫组化。结果第2,4,8周与对照组相比骨折并脑外伤组、脑外伤组、骨折组大鼠血清leptin浓度均有显著性增高(P〈0.05);第4周时骨折并脑外伤组大鼠血清leptin升高水平较其它两组明显(P〈0.05);从第8周开始,手术3组大鼠血清leptin浓度都有所下降,但骨折并脑外伤组leptin浓度仍明显高于其它两组(P〈0.05);第12周各组之间无统计学差异(P〉0.05)。脑外伤合并骨折组形成骨痂体积在X线片上高于单纯股骨骨折组,差异有显著性(P〈0.05)。骨折合并脑外伤组中骨痂组织leptin免疫组化染色平均阳性细胞数比骨折组有显著性增加(P〈0.05)。结论伴有脑外伤的骨折愈合过程中,在一定时间内血清leptin水平升高,骨折愈合加快,提示Leptin是脑外伤后骨折愈合加速的因素之一。
