冠心病维持性血液透析患者心率减速力与心率变异性的分析

目的 测定心率减速力与心率变异性指标,分析维持性血液透析的冠心病患者自主神经功能变化.方法选择35例维持性血液透析的冠心病患者,与年龄、性别相匹配的35例肾功能正常的冠心病患者及35例心脏、肾功能正常者进行比较,采集24 h动态心电图的DC值和HRV各指标值进行统计分析.结果三组间DC值、SDNN、rMSSD和pNN50比较,差异具有统计学意义(P＜0.05);维持血液透析组与肾功能正常组 DC、SDNN、rMSSD和pNN50指标低于对照组(P＜0.05);维持血液透析组与肾功能正常组间DC值、SDNN、rMSSD和pNN50比较,差异无统计学意义(P＞0.05).结论维持血液透析的冠心病患者自主神经功能降低,但与肾功能正常的冠心病患者相比差异无统计学意义.

The analysis of deceleration capacity of rate and heart rate variability in coronary heart disease patients with undergoing maintenance hemodialysis

Objective To determinate of deceleration capacity of rate and heart rate variability in coronary heart disease （CHD） patients with undergoing maintenance hemodialysis and to analyze the change of function of the autonomic nerve. Methods 35 cases of CHD with undergoing maintenance hemodialysis, and 35 CHD cases with normal renal function matched with age and sex, another 35 healthy people were selected as control group. The DC and time-domain HRV parameters were measured and analyzed by digitized 24 hours Holter recording. Results DC, SDNN, rMSSD and pNN50 were statistically significant in all 3 groups （P〈O.05）. The DC value of CHD with maintenance hemodialysis group and with normal renal function were lower than the control group （P〈 0.05）. SDNN, rMSSD and pNN50 of maintenance hemodialysis group were lower than those in the control group （P〈0.05）. No statistical difference was detected between the two groups with maintenance hemodialysis group and with normal renal function in DC, SDNN, rMSSD and pNN50 （P〉0.05）. Conclusion The function of autonomic nerves with maintenance hemodialysis in CHD is reduce and vagal tone is visible decrease but no statistical difference was detected between the two groups with maintenance hemodialysis group and with normal renal function in DC, SDNN, rMSSD and pNN50.
