病毒性肝炎患者肝功能综合评价指标的探索

目的结合患者基本体能状态、实验室检查指标和影像学检查三个方面的资料,建立一个肝功能综合评价指数,用于患者基本病情评价和预后估计。方法选择病毒性肝炎患者305例,根据病历记录资料,分别评出基本体能、实验室检验和影像学检查分值,再计算出总分值,各组间进行对比分析。结果急性肝炎、急性重型肝炎、亚急性重型肝炎、慢性重型肝炎及活动性肝硬化患者实验室检验评分较高;重型肝炎基本体能评分较高。病情越重总分值亦越高,各组间比较有显著性或高度显著性差异。结论该肝功能综合评价指标对肝功能状态评价、病情预后及药物治疗疗效评价均有一定价值。

Establishment and application of total hepatic function index in patients with viral hepatitis

Objective Combinating the essential physician check-up,biochemical examination,and image examination,we made up a total hepatic function index.It was used to be detect the disease degree and judge the prognosis of patients with viral hepatitis.Method This study included 304 cases of patients with various viral hepatitis.They were separated to 8 groups,such as acute hepatitis,sub-acute heavy hepatitis,chronic hepatitis(mild,middle,severe),chronic heavy hepatitis,silent cirrhosis,and active cirrhosis.Every group was scored through essential physician check-up,biochemical examination,and image examination.Adding up the scores of three parts,the score of total hepatic function index were formed.Result There were significant difference between mild liver disease and heavy liver disease.The score of total hepatic function index was well correlated with degree of the various viral hepatitis.Conclusions This hepatic function index is useful for evaluating,and predicting the patients with viral hepatitis than Child-Pugh class.
