电解可脱性铂金弹簧圈栓塞治疗颅内动脉瘤

目的：评价电解可脱性铂金弹簧圈（ＧＤＣ）栓塞颅内动脉瘤的效果。方法在局麻或全麻及全身抗凝下，经导引导管将微导管头端送入动脉瘤腔，用ＧＤＣ逐步将瘤腔填塞满意。术前、术中、术后造影对比，随访１－１６个月。结果２２例共２３个动脉瘤栓塞２１例，其中１６个１００％栓塞（７６２％），４个９０％栓塞（１９．０％），１个５０％栓塞（４．８％）；１个伴发动脉瘤未作处理；１个栓塞失败。死亡１例，余术后随访无ＳＡＨ发生

Endovascular Treatment of Intracranial Aneurysms with Guglielmi Detachable Coils

Objective To assess the effect of intracranial aneurysm embolization with Guglielmi detachable coils(GDC). Methods Under local or general anesthesia and after systematic anticoagulation, a micro-catheter was navigated into the aneurysm through a guide catheter. Then, the aneurysm was embolized with GDCs which was introduced by the micro-catheter. Angiography was performed before, durng and after embolization. Patients were followed for 1 to 16 monthes. Results 21 of 23 aneurysms in 22 cases were embolized by GDCs. 100% occlusion was achieved in 16 aneurysms ( 76.2 %), 90% in 4(19%) and 50% in 1(4.8%). One case failed to embolization. And a coincidental unruptured aneurysm was not embolized. One case died, and no rebleeding occured in this group after treatment. 3 of the 4 cases presented with oculomotor paralysis were completely recovered, another case was improved. Conclusion Embolization of intracranial aneurysms with GDCs is safe, effective and reliable. It has opened a new field for the treatment of intracranial aneurysm.
