老年大肠癌116例临床特点及外科治疗体会

目的:探讨老年大肠癌临床特点及外科治疗的有关问题。方法:回顾性分析2000年1月～2006年1月116例70岁以上行外科治疗的大肠癌患者临床资料。结果:老年大肠癌发病率有逐年上升趋势,以中晚期病例居多,因临床表现不典型易误诊,误诊率达62.1%。本组肠梗阻发生率11.2%,67.2%的有1种以上并存病。115例行手术治疗,根治性切除率70.4%,术后并发症发生率35.7%,无手术死亡病例。结论:手术切除是老年大肠癌的理想治疗方法,虽然并存病增加了手术风险,但术前全面了解病情,加强围手术期处理,可提高手术成功率、降低并发症发生率。

Clinical Features of Colorectal Carcinoma and Surgical Treatment in Elderly Patients :Analysis of 116 Cases

Objective:To study clinical features of colorectal carcinoma and surgical treatment in elderly patients.Methods:Retrospectively summarizing the experience on surgical treatment for 116 patients over 70 years of age with colorectal carcinoma from 2000 to 2006.Results:The misdiagnosis rate was 62.1%,the percentage of intestinal obstruction was 11.2%,most of the patients had other diseases(67.2%),radical resection rate 70.4%,surgical complication rate was 35.7%.There was no surgical death.Conclusion:The operation is an ideal way to treat the elderly patients with colorectal carcinoma.The coexisting diseases increase the risk of surgical procedures,but complete assessment of the patient's general health and strengthening perioperative management of the coexisting diseases can increase the operation success and decrease the mortality and complication.
