静脉回流重建在指尖离断再植中的意义

目的回顾分析静脉回流重建在指尖离断再植中的作用和意义。方法对52例66指指尖离断行再植，均尽量多地吻合静脉；如术后出现静脉栓塞，配合指甲或指腹放血治疗。结果66指成活59指，成活率89．4％。经3-9月随访，指甲宽度和长度平均分别是对侧指的90．5％和95．4％，拇指末节和其余指远指间关节的活动范围平均分别是对侧的92％和81％，指腹两点辨别觉平均6．9mm。结论重视静脉回流通路的重建是指尖再植成功的关键。

Significance of reconstruction of venous reflow in fingertip replantation

Objective To retrospectively analyze the effect and significance of venous reflow reconstruction in replantation of fingertip amputation.Methods 66 fingertips in 52 cases of severed fingertip were involved in the study. Venous anastomosis was performed as much as possible. When venous congestion occurred postoperatively, continuously external bleeding was carried out. Results Of 66 fingertips replantation, 59 were survived, with a success rate of 89.4%. For 3 to 9 months following-up, mean nailbed widths and distal-segment lengths were 90.5% and 95.4% of normal respectively. Average range of motion of the thumb-interphalangeal or finger-dital interphalangeal joints were 92% and 81% of normal respectively. The mean static 2-point discrimination of two points was 6.9mm. Conclusion Strengthening the reconstruction of venous reflow is the key to improve success rate of fingertip replantation.
