高血压性脑出血脑疝的评估及干预的意义

目的分析高血压性脑出血脑疝的临床表现及诊断要点,总结干预措施对患者预后的影响。方法回顾分析2005年2月—2011年8月收治的高血压性脑出血并发脑疝107例患者的临床资料,其中并发小脑幕切迹疝57例、脑中心疝37例、枕骨大孔疝13例。结果 (1)死亡32例;(2)ADL:Ⅰ级6例;Ⅱ级24例;Ⅲ级35例;Ⅳ级7例;Ⅴ级3例。结论正确评估高血压性脑出血病程中出现的脑疝并采取有效的干预措施是非常重要的,关系到评估病情、制定治疗方案及判断患者的预后。

Significance of evaluation and intervention for hypertensive cerebral hemorrhage with cerebral hernia

Objective To analyze the clinical manifestations and diagnostic points of hypertensive cerebral hemorrhage with cerebral hernia,summarize the effect of intervention on the prognosis of patients. Methods clinical data of 107 patients with hypertensive cerebral hemorrhage and cerebral hernia from February 2005 to August 2011 were retrospective analyzed.There were 57 cases of transtentorial hernia,37 cases of brain central hernia and 13 cases of cerebellar tonsillar hernia. Results(1) 32 death cases;(2) ADL: 6 cases of gradeⅠ,24 cases of gradeⅡ,35 cases of grade Ⅲ,7 cases of grade Ⅳ and 3 cases of grade Ⅴ. Conclusion Estimate the cerebral hernia following hypertensive cerebral hemorrhage correctly and take effective intervention are very important.It is related to estimating patient's condition,making therapeutic plan and determining the prognosis of patients.
