锁定钢板外置固定治疗儿童肱骨干骨折

目的探讨锁定钢板外置固定治疗儿童肱骨干骨折的临床效果。方法回顾性分析2015年1月至2019年12月我院收治的45例儿童肱骨干骨折患者资料,其中行锁定钢板内置固定24例,行锁定钢板外置固定21例,术后随访6～8个月,比较两组的手术时间、切口长度、术中出血量、住院时间、骨折愈合时间及术后并发症发生率等各项指标。结果锁定钢板外置固定组的手术时间、切口长度、术中出血量、住院时间、骨折愈合时间及术后并发症总发生率等各项指标均优于锁定钢板内置固定组,差异有统计学意义(P0.05),两组患儿术后肩关节及肘关节功能比较,差异无统计学意义(P0.05)。结论锁定钢板外置固定治疗儿童肱骨干骨折切口小,医源性损伤轻、住院时间短、骨折愈合快、并发症少、骨折愈合后可在门诊取出外置钢板,是一种可供选择的手术方式。

External fixation with locking plate in the treatment of pediatric humeral shaft fractures

Objective To explore the clinical effect of external fixation with locking plate in the treatment of pediatric humeral shaft fractures.Methods The data of 45 children patients with humeral shaft fractures who were admitted to our hospital from January 2015 to December 2019 were retrospectively analyzed.Among them,24 cases were given internal fixation with locking plates,and 21 cases were given external fixation with locking plates.The postoperative follow-up time was 6-8 months.The operation time,incision length,intraoperative blood loss,length of hospital stay,fracture healing time and postoperative complication rate were compared between the two groups.Results The operation time,incision length,intraoperative blood loss,length of hospital stay,fracture healing time and postoperative complication rate of the locking plate external fixation group were better than those of the locking plate internal fixation group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the functions of the shoulder and elbow joints between the two groups of children patients after operation(P>0.05).Conclusion Locking plate external fixation in the treatment of pediatric humeral shaft fractures has a small incision,light iatrogenic injury,short hospital stay,fast fracture healing,and fewer complications.After the fracture has healed,the external plate can be removed at clinic visits.This treating method can be used as an alternative surgical method.
