不同时辰中药保留灌肠对治疗溃疡性结肠炎的随机对照研究

目的:探讨不同时辰中药保留灌肠对治疗溃疡性结肠炎的有效性及差异性。方法:选择符合纳入标准的溃疡性结肠炎住院患者120例,采用简单数字的随机方法随机分为治疗组、对照1组、对照2组,每组40例。3组均给予美沙拉嗪肠溶片1 g,口服,3次/d的基本治疗,对照1组给予上申时(13:00~15:00)中药保留灌肠;对照2组给予上亥时(21:00~23:00)中药保留灌肠;治疗组给予上卯时(5:00~7:00)中药保留灌肠。观察3组患者治疗前后细胞因子水平、中医症状、大肠粘膜积分变化。结果:治疗后治疗组与对照1组、对照2组在细胞因子(Hs-CRP、IL-6、TNF-α)水平、中医症状积分(腹痛、腹泻、脓血便)、大肠粘膜积分方面比较,有显著差异(P0.05);治疗组临床疗效总有效率为92.50%,对照1组临床疗效总有效率为80.00%,对照2组临床疗效总有效率为77.25%,治疗组与对照1组、对照2组在临床疗效总有效率方面比较有显著差异(P0.05)。结论:中药保留灌肠能明显改善溃疡性结肠炎的细胞因子水平及大肠黏膜情况,缓解临床症状;但是不同时辰灌肠导致其临床疗效有差异性,选卯时中药保留灌肠更有利于溃疡性结肠炎的治疗。

A Randomized Controlled Study on Treatment of Ulcerative Colitis by Retention Enema with Chinese Herbal Medicine at Different Time

Objective:To investigate the efficacy and the difference of retention enema with traditional Chinese medicine in treating ulcerative colitis.Methods:120 patients with ulcerative colitis who met the inclusion criteria were selected and randomly divided into treatment group,control group and control group (n =2),with each group of 40 cases in each group.The 3 groups were given basic treatment of oral 3g/d,Mesalazin Enteric-coated Tablets 1g,the control group was given on the 1 (13:00-15:00) of Chinese medicine retention enema;the control group 2 given Haishi (21:00-23:00) retention enema;treatment group was given by (5:00-7:00) retention enema with traditional Chinese medicine.The 3 groups of patients before and after treatment of cytokines,TCM symptoms,changes in colorectal mucosal score.Results:After treatment,the treatment group and the control group 1 and control group 2 on cytokines (Hs-CRP,IL-6,TNF-alpha) level,TCM symptoms (abdominal pain,diarrhea,pus and blood stool),colorectal mucosa score,there was significant difference (P ＜ 0.05);the clinical efficacy of the treatment group the total effective rate was 92.50％,control the clinical curative effect of 1 groups of the total effective rate was 80％,compared 2 groups of clinical curative effect the total effective rate was 77.25％,the treatment group and the control group 1,control group 2 in the clinical curative effect of the total efficiency had significant difference (P ＜ 0.05).Conclusion:Enema can significantly improve the level of cytokines in ulcerative colitis and colorectal mucosa retention medicine,relieve clinical symptoms;but different time leads to the clinical curative effect of enema have difference,selected by Retention Enema with traditional Chinese medicine is more conducive to the treatment of ulcerative colitis.
