甲基强的松龙在颈脊髓压迫患者围手术期中的疗效观察

目的观察甲基强的松龙(MP)在颈脊髓受压患者围手术期治疗中的作用。方法回顾性分析颈椎管狭窄症患者37例,其中24例作为用药组,采用一期后路或前路椎管减压手术治疗,减压前、后10min分别给予MP500mg静脉快速滴入,术后第1天起MP按照200mg、200mg、80mg、80mg逐日减量,共应用4天。此外选取同期13例患者作为对照组,对术前、术后5天、术后1月及术后3月时两组患者的脊髓功能进行评分。结果两组患者无围手术期死亡病例,随访0.3-2.5年;术前ASIA评分用药组与对照组无显著性差异(P>0.05);术后5天、术后1月及术后3月两组ASIA评分同期比较差异有显著性(P<0.05)。结论在颈脊髓受压患者的围手术期应用甲基强的松龙可有效促进脊髓神经损害的功能恢复。

An efficacy evaluation of methylprednisolone in cases with spinal cord compression during perioperative period

Objective To investigate the effect of methylprednisolone(MP)on patients with cervical spinal cord injury during perioperative period.Method Retrospective study were carried out on 37 cases with cervical spinal cord injury,ncluding 24 cases in drug group,which were performed with anterior or posterior canal decompression in one stage posterior or anterior approach,and were administrated with 500 mg MP in less than 10 min before and after decompression,with 200 mg,200mg,80mg,80 mg MP day by day.13 cases in control group were not administrated with MP.The neurological function between two groups was compared in term of ASIA score.Result No death was noted in two groups.All cases were followed up for 0.3-2.5 years.Average ASIA score of two groups before surgery was no remarkable significance(P>0.05).There were remarkable significance(P<0.05)in two group in postoperation and after 1 month and 3months.Conclusion Methylprednisolone is effective for the recovery of cervical spinal cord injury during perioperative period.
