经外侧裂-岛叶路径显微手术治疗基底节区脑出血

摘    要:目的探讨经外侧裂-岛叶入路,显微手术治疗基底节区脑出血的应变策略及疗效。方法回顾性分析37例基底节区脑出血患者的临床资料,其中28例为高血压脑出血;6例为动静脉畸形,2例为大脑中动脉瘤,1例为海绵状血管瘤,均采用经外侧裂显微手术治疗。结果脑内血肿基本清除,动脉瘤全夹闭,动静脉畸形及海绵状血管瘤全切。术后根据改良Rankin评分,0-3分占78%。结论经外侧裂显微手术治疗基底节区脑出血,是一种安全,行之有效的手术方法。

Abstract:Objective To investigate the strategies and efficacy of transsylvian-transinsular microsurgery in the treatment of basal ganglia hemorrhage.Methods A retrospective analysis was performed based on the clinical data of 37 patients with basal ganglia hemorrhage, including 28 patients with hypertensive cerebral hemorrhage, 6 patients with arteriovenous malformation, 2 patients with middle cerebral artery aneurysms, and one patient with cavernous angioma. All patients were treated with transsylvian microsurgery.Results Intracerebral hematoma was completely removed; aneurysms were clipped; arteriovenous malformation and cavernous angioma were completely excised. According to the modified Rankin scale, 78% of patients had a score of 0-3 after surgery.Conclusions The transsylvian-transinsular microsurgery is a safe and effective surgical approach for basal ganglia hemorrhage.
