超声对甲状腺微小乳头状癌的诊断价值

目的探讨超声诊断甲状腺微小乳头状癌的价值。方法分析经手术和病理证实的46例(57个癌结节)甲状腺微小乳头状癌患者的超声表现,其超声特征与58个≤1 cm的甲状腺良性结节进行对照分析。结果甲状腺微小乳头状癌超声表现为低回声(94.7%)、实性成分(100%)、形态不规则(68.4%)、纵横比>1(31.6%)、边界不清晰(61.4%)、微小钙化(66.7%)、内部血流(28.1%),以及颈部及锁骨上可探及肿大淋巴结(36.8%),其上述超声特征的显示率明显高于甲状腺良性结节,差异有统计学意义(P<0.05)。结论超声对诊断甲状腺微小乳头状癌具有重要价值。

Value of ultrasonography in diagnosis of papillary thyroid microcarcinoma

Objective To investigate the value of ultrasonography in diagnosis of papillary thyroid microcarcinoma. Methods Ultrasonic manifestations of 46 patients （57 cancerous nodules） with papillary thyroid microcarcinoma confirmed by surgery and pathology were analyzed retrospectively, and then compared with those of 58 benign thyroid nodules with the diameter of lcm or less. Results The detection rates of multiple ultrasonic manifestations, including hypoechoic （94.7%）, solid lesion（ 100% ）, irregular shape（68.4% ）, A/T〉 1 （ 31.6% ）, ill-defined edge （61.4% ）, microcalciffcation（66.7% ）, internal blood flow pattern （28.1%） and metastatic lymph nodes （36.8%）were significantly higher in the cancerous-nodule group than those in benign-nodule group. There was significant difference between the two groups （P〈0.05）. Conclusion Uhrasonography has important value in diagnosis of papillary thyroid microcarcinoma.
